Death of worker prompted OSHA investigation
Agtegra Cooperative has been issued three citations and fined $14,000 after an accident at a facility in Roscoe that resulted in an employee’s death.
The incident was reported around 9:45 a.m. on March 29 as a 52-year-old male employee was working with a grain conveyor. His death led to an investigation by the Occupational Safety and Health Administration.
OSHA’s initial report was filed last month after its investigation was finished, but has since been updated. It describes what happened in disturbing detail.
In short, an employee was trying to move grain on a conveyor, but the door wouldn’t open. He tried to troubleshoot the gate, but couldn’t so he needed to get in the conveyor to do more work, according to the report.
